Patient characteristics and predictors of mortality in 470 adults admitted to a district general hospital in England with Covid-19

Understanding risk factors for death from Covid-19 is key to providing good quality clinical care. We assessed the presenting characteristics of the ‘first wave’ of patients with Covid-19 at Royal Oldham Hospital, UK and undertook logistic regression modelling to investigate factors associated with death. Of 470 patients admitted, 169 (36%) died. The median age was 71 years (interquartile range 57–82), and 255 (54.3%) were men. The most common comorbidities were hypertension (n = 218, 46.4%), diabetes (n = 143, 30.4%) and chronic neurological disease (n = 123, 26.1%). The most frequent complications were acute kidney injury (AKI) (n = 157, 33.4%) and myocardial injury (n = 21, 4.5%). Forty-three (9.1%) patients required intubation and ventilation, and 39 (8.3%) received non-invasive ventilation. Independent risk factors for death were increasing age (odds ratio (OR) per 10 year increase above 40 years 1.87, 95% confidence interval (CI) 1.57–2.27), hypertension (OR 1.72, 95% CI 1.10–2.70), cancer (OR 2.20, 95% CI 1.27–3.81), platelets <150 × 103/μl (OR 1.93, 95% CI 1.13–3.30), C-reactive protein ≥100 μg/ml (OR 1.68, 95% CI 1.05–2.68), >50% chest radiograph infiltrates (OR 2.09, 95% CI 1.16–3.77) and AKI (OR 2.60, 95% CI 1.64–4.13). There was no independent association between death and gender, ethnicity, deprivation level, fever, SpO2/FiO2, lymphopoenia or other comorbidities. These findings will inform clinical and shared decision making, including use of respiratory support and therapeutic agents.

The Working Party has produced this report in order to prompt readers to engage at an early stage in InsurTech projects, through considering (i) the full range of risks associated with InsurTech developments, (ii) the lifecycle of an InsurTech venture and how any risk considerations may vary over this lifecycle and (iii) the extent to which InsurTech ventures align with risk strategy and risk appetite.
The report contains practical guidance for actuaries, risk professionals, insurance companies and their Boards on these considerations, and can be used to facilitate appropriate questioning, to help ensure that InsurTech-related business decisions are fully cognisant of the risk management issues and to help ensure the success of projects.
The Working Party developed this guidance having carried out an industry survey on a number of risk management topics relating to InsurTech, as well as having carried out interviews with a number of relevant senior stakeholders across the insurance industry, in order to better understand current sentiment and how risk management plays a part when considering opportunities in InsurTech. The Working Party views on the findings from these activities are summarised in the report.

The preparation of green rusts from sulphate solutions and representative Mössbauer spectra are described. As the samples oxidized readily, attention focused on the Mössbauer parameters at liquid nitrogen and helium temperatures. The spectra recorded at 77 K could be fitted satisfactorily with one ferrous iron quadrupole doublet with a separation of 2·93 mms−1 and one ferric iron quadrupole doublet with a separation of 0·45 mms−1. In some spectra a ferric iron magnetic hyperfine of strength 49·2 T was also apparent. At 4·2 K, the ferrous iron exhibited a hyperfine splitting with a field of 12·4 T whilst the ferric iron exhibited a hyperfine splitting with a field of strength 50·4 T. The ratio of ferrous to ferric ions was 2·25 ± 0·25 at 77 K and at 4·2 K, and ∼1·6 with a large variation at room temperature. The liquid helium spectra did not always give a good chi-squared fit, the main reason being attributed to relaxation. The line-width of the ferrous iron site at 77 K is slightly larger than that for iron metal and could be explained by a variation in the number of near Fe3+ neighbours at different Fe2+ sites, consistent with the assumption that the ferrous iron site is in the hydroxide sheet. The effect of different numbers of Fe2+ and Fe3+ neighbours probably contributed to the increase in line-widths at 4·2 K compared with those at 77 K. The ferrous iron doublet is marginally different to those of chloride and hydroxy-carbonate green rusts and the aluminium analogues.

Cohort effects are important factors in determining the evolution of human mortality for certain countries. Extensions of dynamic mortality models with cohort features have been proposed in the literature to account for these factors under the generalised linear modelling framework. In this paper we approach the problem of mortality modelling with cohort factors incorporated through a novel formulation under a state-space methodology. In the process we demonstrate that cohort factors can be formulated naturally under the state-space framework, despite the fact that cohort factors are indexed according to year-of-birth rather than year. Bayesian inference for cohort models in a state-space formulation is then developed based on an efficient Markov chain Monte Carlo sampler, allowing for the quantification of parameter uncertainty in cohort models and resulting mortality forecasts that are used for life expectancy and life table constructions. The effectiveness of our approach is examined through comprehensive empirical studies involving male and female populations from various countries. Our results show that cohort patterns are present for certain countries that we studied and the inclusion of cohort factors are crucial in capturing these phenomena, thus highlighting the benefits of introducing cohort models in the state-space framework. Forecasting of cohort models is also discussed in light of the projection of cohort factors.

An unexpected increase in gastroenteritis cases was reported by healthcare workers on the KwaZulu-Natal Coast, South Africa, January 2017 with >600 cases seen over a 3-week period. A case–control study was conducted to identify the source and risk factors associated with the outbreak so as to recommend control and prevention measures. Record review identified cases and controls and structured-telephonic interviews were conducted to obtain exposure history. Stool specimens were collected from 20 cases along with environmental samples and both screened for enteric pathogens. A total of 126 cases and 62 controls were included in the analysis. The odds of developing gastroenteritis were 6.0 times greater among holiday makers than residents (95% confidence interval (CI) 2.0–17.7). Swimming in the lagoon increased the odds of developing gastroenteritis by 3.3 times (95% CI 1.06–10.38). Lagoon water samples tested positive for norovirus (NoV) GI.6, GII.3 and GII.6, astrovirus and rotavirus. Eleven (55%) stool specimens were positive for NoV with eight genotyped as GI.1 (n = 2), GI.5 (n = 3), GI.6 (n = 2), and GI.7 (n = 1). A reported sewage contamination event impacting the lagoon was the likely source with person-to-person spread perpetuating the outbreak. Restriction to swimming in the lagoon was apparently ineffective at preventing the outbreak, possibly due to inadequate enforcement, communication and signage strategies.

Objective
To characterize the major components of the contemporary Inuit diet and identify the primary sources of energy and essential nutrients.
DesignDietary data were derived from the 24 h recall collected by the Inuit Health Survey (IHS) from 2007 to 2008. The population proportion method was used to determine the percentage contribution of each group. Unique food items/preparations (ninety-three country foods and 1591 market foods) were classified into eight country food groups and forty-one market food groups. Nutrient composition of each food item was obtained from the Canadian Nutrient File.
SettingThirty-six communities across three Inuit regions of northern Canada.
SubjectsA representative sample (n 2095) of non-pregnant Inuit adults (≥18 years), selected through stratified random sampling.
ResultsDespite their modest contribution to total energy intake (6·4–19·6 %, by region) country foods represented a major source of protein (23–52 %), Fe (28–54 %), niacin (24–52 %) and vitamins D (up to 73 %), B6 (18–55 %) and B12 (50–82 %). By contrast, the three most popular energy-yielding market foods (i.e. sweetened beverages, added sugar and bread) collectively contributed approximately 20 % of total energy, while contributing minimally to most micronutrients. A notable exception was the contribution of these foods to Ca (13–21 %) and vitamins E (17–35 %) and C (as much as 50 %). Solid fruits were consumed by less than 25 % of participants while vegetables were reported by 38–59 % of respondents.
ConclusionsCountry foods remain a critical dimension of the contemporary Inuit diet.

This paper explores and develops alternative statistical representations and estimation approaches for dynamic mortality models. The framework we adopt is to reinterpret popular mortality models such as the Lee–Carter class of models in a general state-space modelling methodology, which allows modelling, estimation and forecasting of mortality under a unified framework. We propose alternative model identification constraints which are more suited to statistical inference in filtering and parameter estimation. We then develop a class of Bayesian state-space models which incorporate a priori beliefs about the mortality model characteristics as well as for more flexible and appropriate assumptions relating to heteroscedasticity that present in observed mortality data. To study long-term mortality dynamics, we introduce stochastic volatility to the period effect. The estimation of the resulting stochastic volatility model of mortality is performed using a recent class of Monte Carlo procedure known as the class of particle Markov chain Monte Carlo methods. We illustrate the framework using Danish male mortality data, and show that incorporating heteroscedasticity and stochastic volatility markedly improves model fit despite an increase of model complexity. Forecasting properties of the enhanced models are examined with long-term and short-term calibration periods on the reconstruction of life tables.

Outbreak of giardiasis associated with a community drinking-water source

Giardiasis is a common waterborne gastrointestinal illness. In 2007, a community giardiasis outbreak occurred in New Hampshire, USA. We conducted a cohort study to identify risk factors for giardiasis, and stool and environmental samples were analysed. Consuming tap water was significantly associated with illness (risk ratio 4·7, 95% confidence interval 1·5–14·4). Drinking-water samples were coliform-contaminated and a suspect Giardia cyst was identified in a home water filter. One well was coliform-contaminated, and testing indicated that it was potentially under the influence of surface water. The well was located 12·5 m from a Giardia-contaminated brook, although the genotype differed from clinical specimens. Local water regulations require well placement at least 15 m from surface water. This outbreak, which caused illness in 31 persons, represents the largest community drinking-water-associated giardiasis outbreak in the USA in 10 years. Adherence to well placement regulations might have prevented this outbreak.
The Diffusion of Radionuclides through Waste Encapsulation Grouts

United Kingdom Nirex Limited develops and advises on safe, environmentally sound and publicly acceptable options for the long-term management of radioactive waste. One option Nirex has developed is a phased geological repository concept for intermediate level waste and some low level wastes that makes use of a combination of engineered and natural barriers. Physical containment of radionuclides would be achieved by immobilisation and packaging of wastes (mostly) in stainless steel containers.
Existing models of the migration of dissolved radionuclides from packaged wastes suggest that radionuclide release is determined largely by the rate of diffusion through the encapsulation grout used to immobilise the waste. The use of such models requires diffusion coefficient data for radionuclides in waste encapsulation grouts. This paper describes a programme of through-diffusion experiments, and modelling interpretation, aimed at deriving diffusion coefficients for some radionuclides in two types of encapsulation grout.
An intrinsic diffusion coefficient of HTO of around 1×10−13 to 2×10−13 m2s−1 was determined for a 3:1 mix of blast furnace slag to ordinary Portland cement, compared to around 4×10−13 to 5×10−13 m2s−1 for a 3:1 mix of pulverised fuel ash to ordinary Portland cement. These values are lower than that assumed for a non-sorbing radionuclide in an earlier modelling exercise. Porosity values around 0.3 were obtained in each case. For 36Cl as chloride, the experiments showed no significant breakthrough over the experimental timescale of about one year, suggesting an intrinsic diffusion coefficient below 5×10−13 m2s−1. One possibility is that chlorine-containing solids are precipitating in the cement. An intrinsic diffusion coefficient for 137Cs in the 3:1 mix of pulverised fuel ash to ordinary Portland cement of 4×10−15 m2s−1 was estimated, significantly lower than that determined for HTO.
The results from three of the sixteen experiments could not be fitted with a simple diffusion model, and for a further five experiments there was some doubt as to whether simple diffusion behaviour had been observed. It is suggested that this may have been due to cracks in the grouts that were sufficiently large to affect the diffusion properties of the grouts, although none was visible to the naked eye. Cracking of the waste encapsulation grouts could provide a mechanism for enhanced migration of radionuclides from waste packages, compared with diffusion in a homogeneous porous medium alone.

We present the results of a study of the evolution of the gas and dust in disks around T Tauri, Herbig Ae and Vega-like stars. We observed the two lowest rotational lines of H2 with the ISO-SWS as well as 12CO 3–2 and 13CO 3–2 with the JCMT, and CO 6–5 with the CSO. The H2 lines trace the warm (∽ 100 K) inner region whereas the CO lines probe the colder outer disks. Substantial amounts of H2 have been detected toward T Tauri and Herbig Ae stars, and, surprisingly, also around three Vega-like objects (49 Cet, HD 135344 and β Pictoris). In contrast with previous conclusions derived from CO data, a significant mass of warm gas is found to persist up to ages of several tens of Myrs, suggesting that slow formation of gas-rich giant planets is possible.
CYANIXIA, A NEW GENUS FOR THE SOCOTRAN ENDEMIC BABIANA SOCOTRANA (IRIDACEAE–CROCOIDEAE)

The sub-Saharan and mainly western southern African genus Babiana (Iridaceae–Crocoideae) is morphologically diverse, containing some 80 species. The Socotran B. socotrana, which differs from all other members of the genus inter alia in its trisulcate pollen grains, basic chromosome number, x=10, corms of axillary origin, and globose, colliculate–tuberculate seeds, is here excluded from Babiana and referred to the new genus Cyanixia. Other Babiana species have monosulcate pollen grains with a two-banded operculum, a basic chromosome number of x=7, terminal corm ontogeny, and derived pear-shaped seeds with a smooth glossy surface. Cyanixia socotrana was previously assigned to Babiana because of superficial similarities in morphology, including plicate leaves and blue flowers, thought at the time to be bilabiate, although now known to be actinomorphic and rotate. DNA sequence analysis using the chloroplast gene matK confirms that Babiana is a monophyletic assemblage, whereas B. socotrana is sister to the Lapeirousia/Savannosiphon clade of sub-Saharan Africa. The latter shares axillary corm ontogeny with the southern African Micranthus, Thereianthus, and Watsonia, in contrast to the more common situation in Crocoideae (including Babiana) in which the corm develops from the base of the flowering stem. The tropical African Zygotritonia, which also has axillary corm development, shares with Cyanixia a trisulcate pollen grain, but it differs markedly from that genus in its small, zygomorphic flower, undivided style, and basic chromosome number x=8. Cyanixia is here described as a new monotypic genus of Crocoideae, probably most closely allied to Savannosiphon, also a monospecific genus.
Conductivity and Ftir Measurements of the Hydrogen Content of Heat Treated Diamond Films

The content of bonded hydrogen in hot filament grown diamond films was determined by Fourier transform Infrared (FTIR) measurements before and after annealing. The quantity of bonded hydrogen was found to remain unchanged on annealing in diamond films with high amounts of microcrystalline diamond and amorphous carbon. The hydrogen content was found to decrease on annealing ∼9 times in diamond film of good crystalline quality. The change of the bulk hydrogen content did not seem to be linked to changes in resistivity of the samples.
